چکیده

Carotid arterial dissections may result from spontaneous or traumatic causes. Postcoital arterial dissections have been reported in both the vertebral and coronary arteries. We report a rare case of spontaneous dissection from the common carotid artery to the internal and external carotid arteries after sexual intercourse, leading to fulminant middle cerebral artery (MCA) territory infarct. Although we restored the flow in the proximal carotid artery with stenting, brain swelling with hemorrhagic transformation was still noted in the region of the right MCA.

Therapeutic Decision Making in Acute Stroke due to Carotid Artery Dissection: A Potential Role for Percutaneous Vascular Intervention following Intravenous Thrombolysis

Internal carotid artery dissection (ICAD) is an important cause of acute ischemic stroke in younger patients. Potential acute treatments include anticoagulation, intravenous thrombolysis (IVT), and endovascular thrombectomy (ET). We report a case where the use of IVT followed by ET resulted in a good clinical outcome in a patient with tandem internal carotid and middle cerebral artery occlusion...

Aortic Dissection Complicated with Fatal Cerebral Infarction: Case Report and Review of Literatures

Acute aortic dissection is a life-threatening condition requiring immediate assessment and therapy. Rarely, aortic dissection involves carotid arteries and manifest cerebral infarction. Here, we report a case of aortic dissection complicated with fatal cerebral infarction. A 83-year-old man, who suddenly suffered consciousness disturbance and right hemiparesis, was transferred to our hospital f...

Concurrent Ruptured Pseudoaneurysm of the Internal Carotid Artery and Cerebral Infarction as an Initial Manifestation of Polycythemia Vera

The most common neurologic manifestations of polycythemia vera (PV) are cerebral infarction and transient ischemic attacks, while cerebral hemorrhage or intracranial dissection has been rarely associated with PV. Here we report the first case of a 59-year-old patient with intracranial supraclinoid internal carotid artery (ICA) dissection causing cerebral infarction and concomitant subarachnoid ...

Cortex-sparing infarction in triple cervical artery dissection following chiropractic neck manipulation

BACKGROUND Multivessel cervical dissection with cortical sparing is exceptional in clinical practice. CASE PRESENTATION A 55-year-old man presented with acute-onset neck pain with associated sudden onset right-sided hemiparesis and dysphasia after chiropractic manipulation for chronic neck pain. RESULTS AND DISCUSSION Magnetic resonance imaging revealed bilateral internal carotid artery dis...

Multiple reversible episodes of subcortical ischemia following postcoital middle cerebral artery dissection.

BACKGROUND Subcortical transient ischemic attacks are usually seen in individuals with small-vessel disease. They are often refractory to medical treatment and progress to infarction in many cases. PATIENT We describe a young man with an unusual and protracted case of recurrent capsular ischemia due to postcoital middle cerebral artery dissection. CONCLUSION Spontaneous middle cerebral arte...
